Instructions for Completing the STATE OF TEXASAPPLICATION FOR EMPLOYMENT Form 4010E andApplicant EEO Data Form Form 4009EWHAT YOU SHOULD KNOWGeneralForm 4009, Applicant EEO Data Form and the State of Texas Application for Employment, Form 4010, were developed using WordPerfect 5.1 They are Merge documents and were developed for HP III or IV Laser Printers, but will work fine on any Postscript laser printer with scaleable fonts. They can also be easily printed on the new IBM Lexmark (4039) series lasers. Please note that they could not be developed for completion on an HP II because of the different font sizes used. A major benefit of using these forms is that once the document is created and saved, revisions can be rather simple. Use a blank state form, or, use WordPerfect and go through the document 4009 and 4010-1 thru 4010-4 and print a blank copy of each. These documents were created to be used by WordPerfect 5.1 (for DOS), but we believe they can be used by WordPerfect for Windows or newer versions of WordPerfect. The exact commands will differ with different versions.NOTE: Because of WordPerfect's software limitations, you must complete the shaded fields on the application by hand. For example, the merge will not allow you to place "X" in the "yes/no" check boxes. You must complete this part of the form after you complete the merge. Use a ballpoint pen and fill in these boxes before you sign it. Detailed Instructions for Page 1To access Page 1 of the State of Texas Application for Employment:
NOTE: Type of School: You will be prompted to complete two (2) lines of the "Undergraduate Colleges or Universities" and "Graduate Schools" fields. You will also be prompted to complete one (1) line of the "Technical, Vocational, or Business Schools" fields. If you only need to complete one line in each type of school, press the F9 key to move through these fields. Once you have completed the "Technical, Vocational, or Business Schools," this page is ready to be printed. If you need to complete the third line of the "Undergraduate Colleges or Universities" and "Graduate Schools" fields, move the cursor up to those fields and press the Tab key to move from field-to-field. Instructions for Completing Pages 2 through 6Follow the steps outlined above, except at the Primary Document prompt, type in the appropriate page number. For example, the Primary Document name for Page 2 is 4010-2. NOTE: Pages 3 through 6 pertain to employment history. Pages 3 and 4 must be submitted as a standard part of the application. Pages 5 and 6 are continuation pages of 3 and 4. If you don't need them, don't use them. Helpful Hint: Summary of Experience It is easier to F9 past the "Summary of Experience," portion on pages 3 through 4 of the application form. Once past the last of the Merge prompts, move to the very bottom of the last form and type your "Summary of Experience," then "move" the data into that field: ie, Block, Ctrl-F4, Move (not Copy). This is also a good time to experiment with different size or style fonts to make your application really stand out. Also, if your WordPerfect application is running a Timed Backup, you may want to turn off this feature since it sometimes slows down the merge process. To turn off the Timed Backup, do a: SHIFT-F1, 3, 1, 1, No Return Return Return Return Don't forget to turn the "backup" feature back on when you are finished. As mentioned on page 1 of these instructions, you can run these programs from a "floppy" disk, but the Merge take longer. It is much faster to copy the files to your hard drive or to you LAN applications drive. Just be sure and SAVE your files to a different name and don't overwrite the included files!
